Covid Spring for senior 65 and older vaccines available now

COVID-19 [Moderna]: for individuals 12 Years and older, will not have infant and children doses till fall.

COVID-19 [Pfizer]for individuals 12 Years and older, will not have infant and children doses till fall.

COVID-19 [Novavax]: for 12 years and over. We no longer have Novavax for this season.

Influenza (flu)-high dose for seniors only, until next flu season.

RSV: (Arexvy) for those 60 years and older

RSV: (Abrysvo) for pregnant patients 32-36 weeks & 6 days.

COVID-19 vaccines for patients under 3 years of age will be administered in collaboration with Dr. Richard Hilburn, MD (Goals of Care) and his nursing staff.
The vaccine administration will be processed as a medical claim with the health insurer. Some health insurer restrictions may apply.

When scheduling your appointment, please make sure you have the relevant Medical and Prescription insurances cards or Medicare card with you. Below are images sample cards showing information we need you to provide when scheduling your appointment.

Please keep in mind, that you only need to provide your Medicare information if you are Medicare eligible.

If you've previously made an appointment with us or received medication services, we only require your full name, address, email, phone number, and date of birth. Feel free to leave other fields blank for your convenience.
WHAT DO I BRING TO MY APPOINTMENT?

On the day of your appointment please make sure to bring the following:

Valid government issued ID.

Relevant prescription and medical insurance cards (if you have Medicaid or private health insurance).

If you have Medicare, bring the original government issued Medicare card (the red, white, and blue card) in addition to your prescription (Part D) card.
