When scheduling your appointment, please make sure you have the relevant Medical and Prescription insurances cards or Medicare card with you. Below are images sample cards showing information we need you to provide when scheduling your appointment.
Please keep in mind, that you only need to provide your Medicare information if you are Medicare eligible.

WHAT DO I BRING TO MY APPOINTMENT?
On the day of your appointment please make sure to bring the following:

Valid government issued ID.

Relevant prescription and medical insurance cards (if you have Medicaid or private health insurance).

If you have Medicare, bring the original government issued Medicare card (the red, white, and blue card) in addition to your prescription (Part D) card.
