WHO QUALIFIES FOR A SECOND
BOOSTER SHOT?
Non-Immunocompromised eligibility:
A second booster dose of the Pfizer-BioNTech COVID-19 Vaccine or Moderna COVID-19 Vaccine may be administered to individuals 50 years of age and older at least 4 months after receipt of a first booster dose of any authorized or approved COVID-19 vaccine.
Immunocompromised eligibility:
Pfizer: A second booster dose of the Pfizer-BioNTech COVID-19 Vaccine may be administered to individuals 12 years of age and older with certain kinds of immunocompromise at least 4 months after receipt of a first booster dose of any authorized or approved COVID-19 vaccine.
Moderna: A second booster dose of the Moderna COVID-19 Vaccine may be administered at least 4 months after the first booster dose of any authorized or approved COVID-19 vaccine to individuals 18 years of ag

Review time slots and schedule an appointment for either your 1st or 2nd COVID-19 vaccine
MUST BE 12+ AND IF YOU'RE UNDER 18 YOU WILL NEED A PARENT/GUARDIAN
Please arrive at your scheduled appointment time and before coming into the vaccination clinic have a government issued ID as well as health insurance information ready when you check in

If you received your first dose then please register below for a follow up email, which will be sent out 1 week before your 2nd dose is due, and schedule an appointment.

WHAT DO I BRING TO MY APPOINTMENT?
On the day of your appointment please make sure to bring the following:

Valid government issued ID

If you have Medicare, bring the original government issued Medicare card (the red, white, and blue card) in addition to your prescription (Part D) card

If you have Medicaid or private health insurance, please bring the relevant prescription insurance cards



Please note that If you don't have health insurance you will still receive your COVID-19 vaccine shot for free.
The cost of COVID-19 vaccine is paid for by the US government. The cost of administration is covered by Medicare, Medicaid or private health insurance. The out of pocket cost is always $0 under all circumstances!
