Review time slots and schedule an appointment for either your 1st, 2nd or 3rd COVID-19 vaccine
IF YOU'RE UNDER 18 YOU WILL NEED A PARENT/GUARDIAN
Please arrive at your scheduled appointment time and before coming into the vaccination clinic have a government issued ID as well as health insurance information ready when you check in

If you received your first dose then please register below for a follow up email, which will be sent out 1 week before your 2nd dose is due, and schedule an appointment.

WHAT DO I BRING TO MY APPOINTMENT?
On the day of your appointment please make sure to bring the following:

Proof of child's age (passport, birth certificate, ID)

Valid government issued ID (for parent or legal guardian)

If you have Medicare, bring the original government issued Medicare card (the red, white, and blue card)

If you have Medicaid or private health insurance, please bring the relevant insurance cards
Please note that If you don't have health insurance you will still receive your COVID-19 vaccine shot for free.
The cost of COVID-19 vaccine is paid for by the US government. The cost of administration is covered by Medicare, Medicaid or private health insurance. The out of pocket cost is always $0 under all circumstances!
WHAT VACCINES DO WE OFFER FOR PEDIATRIC PATIENTS?

We offer Moderna (2 series) for children 6 months - 5 years and Pfizer (3 series) for children 6 months - 4 years.

We will try to accommodate your preference, yet supplies are limited.

My Dr's Pharmacy will not be administering vaccines at the current pharmacy location (412 Elden Street).
